After defeating deadly curses from the previous Pee Nak films, the old crew go on to live their peaceful lives. The hot Thai-Korean star Do Min Joon is busy with fan meetings, while Balloon and First sweep their ways up to become top talent managers.

It sounds like a happy ending. But a new chapter of horror is awaiting them at the Ghost Mask Festival. The Ghost Mask Festival is a local festival to worship the spirits of ancestors, sacred entities that protect their offspring and bring prosperity and abundance. Do Min Joon , Balloon and First happen to be on holiday during the festival. They arrive to follow up on the restoration of a mural painting at a temple where Do Min Joon’s family is a patron.

But the festival of virtue is tainted by bad karma when a mysterious blue-eyed man returns to demand Do Min Joon to fulfill his promise – the promise that he cannot recall when he made it or what it is about exactly.

Do Min Joon is being terrorized by the new, mysterious Pee Nak spirit, while Balloon and First, as his manager, have no choice but to share the burden and prepare to join the ride into another terrifying adventure.

The past has returned to haunt them. The virtue they once made and the sins they once committed will come back to shake their lives. This time, the fear is quadruple.
